Can a participant request further information under the ERISA §404(a)(5) Regulations?

Yes.  The plan administrator is required to furnish to a participant the following information if the participant requests it:

  1. The prospectus or short form or summary prospectuses.
  2. Financial statements or reports relating to designated investment alternatives, if available to the plan.
  3. A statement of the value of a share or unit of a designated investment alternative as well as the date of the valuation.
  4. A list of the assets comprising the portfolio of each designated investment alternative and the value of each such asset or the proportion of the investment which it comprises.
